customer certifies that the State of Texas has issued him/her a

Texas Sales Tax Permit and the customer is responsible to

collect Texas State Sales Tax and pay the same to Texas State Treasury.

Please print following Texas State Resale Certificate, fill it up with your

name, your business name, address and sales tax permit number

exactly as appears on your sales tax permit. Sign the certificate and
